Keep a shared sheet with: task name, prompt version, sample inputs, pass/fail notes, and owner.

45-minute agenda

• 10 min — pick two production tasks that felt flaky last week
• 20 min — run A/B variants on the same five inputs
• 10 min — vote lock or reject; update the vault sheet
• 5 min — assign one cleanup follow-up

Lock rule

Only promote a prompt if two reviewers agree on tone and factual accuracy for the sample set. Archive losers with one-line reasons so you do not rediscover them next month.
